Shaving it down doesn't hurt, as it's dead skin, unless the Dr. accidentally goes a little too deep. All Dr's should be shaving the wart down somewhat before applying treatment (liquid nitro, for example), otherwise you're just treating dead skin and not the wart itself. The same way you are told to exfoliate the area before putting on Compound W or similar.
 
Is liquid nitrogen extremely cold, painful, and uncomfortable?
Yes, it can be. It absolutely will be cold. Pain and level of uncomfortableness is really different for everyone.
 
It burns, but it's bearable. You can be a little sore after too, in the area, but I have very little tolerance for pain and I manage no problem. The joys of competitive swimming. ;)
